If you are needing a FIRE report from the Rendon Fire Department, please send an email to mwinkler@rendonfire.com and some one will get back with you about the needed report. 

If you are needing an EMS report please contact our third party billing company, Emergicon by the following: 

 Attorney requests must be submitted through www.chartswap.com with the EMS service ‘s name as the Provider. 

Non-attorney requests may submit in any of the following ways:

1. Call 972-602-2060 ext. 1611
2. Fax the request (secure and direct) to 800-608-9457
3. Email records@emergicon.com with general information and you will receive a secure email to use for the PHI details
4. Mail the request to PO Box 180446, Dallas, TX  75218.
